All Wentworth Health Partners practices strive to provide their patients the best health care possible in a positive and supportive environment. One of the ways we determine if our providers and staff are meeting patients' needs and expectations is through Press Ganey surveys. A random selection of patients will receive a survey by mail or email within a month of their appointment. These surveys are confidential unless the patient chooses to provide his or her name. 

We value the feedback provided in the surveys and review this data carefully. We appreciate your time and willingness to help us to improve our level of service by completing these surveys when you receive them. Thank you in advance for your feedback. 

Patient Rights

You have a right to safe and appropriate care and services by people who treat you with respect and dignity. Patient Concerns and Feedback

If you have any questions or concerns about the care or services you received, please contact your practice and ask for the practice manager. Your feedback will remain anonymous if you wish, and your patient care will not be affected. 

Patients or family who have concerns about safety or the quality of care provided by Wentworth Health Partners may report these concerns to the Joint Commission. Visit The Joint Commission website for instructions on how to submit a report.
