The Patient & Family Learning Center provides a comfortable and relaxing space for patients and family members to find health information and support. Our staff is specially trained to help you locate the most current and accurate information about your heath or illness.
The Patient & Family Learning Center also hosts a number of Support Groups and runs several free programs geared toward improving the health and wellness of our Seacoast community. These offerings include certified health coaching, the Prevent Type 2 Diabetes Program, and tobacco/nicotine cessation.
Let us help you find your path to better health. Our certifiedhealth coaches can provide you with the knowledge, skills, tools, and confidence to become an active participant in your care and improve all areas of your wellness, including physical activity, nutrition, weight, stress, and life satisfaction. Our health coaches specialize in helping people with chronic conditions, such as diabetes, COPD, obesity, and others. After working with a health coach for 8-12 sessions (in-person or virtually), patients typically reach more of their goals and report feeling more energized and confident as they continue their journey.

One out of three American adults has prediabetes, a condition that puts people at risk of developing type 2 diabetes within five years. The good news: prediabetes can often be reversed. The Patient & Family Learning Center offers the Prevent Type 2 program, developed as part of the National Diabetes Prevention effort and backed by the Centers for Disease Control and Prevention.
As a participant in the Prevent Type 2 program, you will be part of a small group that meets weekly with a trained lifestyle coach to discuss and support goals to eat healthier, move more, reduce stress, and improve sleep. The second half of the program is designed to help individuals maintain the healthy changes they have learned and turn them into healthy habits. Research has shown that these lifestyle changes can reduce a person’s risk of developing type 2 diabetes by half.

We want to help you quit smoking and using other tobacco products like chew, cigars, hookah, and e-cigarettes. Working together with our health coaches and respiratory therapists, we will develop an individualized approach and provide step-by-step guidance to help you reach our goals. We will also coordinate with your medical providers for referrals for medications, nicotine replacement, or other helpful resources.
