Patient and Family Relations

We strive to provide the best possible experience for each patient, but know that we may occasionally fail to meet expectations. If you have a concern related to care or service that you cannot resolve with your health care team, or if you have a suggestion for improved services, please call Wentworth-Douglass Hospital’s Patient and Family Relations office at (603) 740-2823. We will work with you and the care team to respond to your concern.

You may also file a grievance with the State of New Hampshire Department of Health & Human Services by contacting the Office of Legal & Regulatory Services, Bureau of Health Facilities Administration, at 129 Pleasant Street, Concord, NH 03301 or by calling (603) 271-9039 or toll free at (800) 852-3345.

The Joint Commission

Wentworth-Douglass Hospital and our physician practices strive to provide high-quality safe care. Any patient or employee concerns regarding patient safety should be brought to the attention of our clinical leaders and administration. Patients and employees also have the right to contact The Joint Commission regarding patient care and safety. You may report concerns to The Joint Commission by the following methods:

  • At www.jointcommission.org, using the “Report a Patient Safety Event” link in the “Action Center” on the home page of the website
  • By fax to 630-792-5636

Patient and Family Advisory Council

Wentworth-Douglass Hospital’s Patient and Family Advisory Council (PFAC) is a group of strongly motivated patients, family members, and staff who apply firsthand knowledge to improve the experience for our patients, family members, and caregivers. Patients and family members are invited to apply to join our PFAC. Members provide feedback, advise our staff, and promote collaboration between the hospital and the community. The council meets monthly at Wentworth-Douglass Hospital.
